The Friends Investing in Foster Kids (FIFK) Forms page is designed to make it simple and efficient for caregivers, partners, and approved volunteers to access the paperwork needed to request support. Friends Investing in Foster Kids is a 501(c)(3) Non-Profit serving the needs of children in the Child Protection and Permanency Court in Dallas County, and we understand that families and youth often need timely help—without unnecessary steps or confusion. These forms help us respond quickly, track requests responsibly, and ensure resources are directed to the youth and caregivers who need them most.

At this time, our Forms page includes two primary resources: the Adoption Backpack Order Form and the Reimbursement Form. As our programs grow, this page will expand to include additional requests and resources, but our goal will remain the same—clear, supportive, and easy-to-use tools that reduce barriers for those caring for youth in foster care.

Adoption Backpack Order Form

The Adoption Backpack Order Form is used to request an Adoption Backpack for a child or youth who is moving toward permanency through adoption. These backpacks are meant to provide practical items, comfort, and encouragement during an important life transition. For many children, adoption can bring a mix of emotions—hope, excitement, uncertainty, and change happening all at once. A thoughtfully prepared backpack can help a child feel celebrated and supported, and it can provide essentials that caregivers may not have time or resources to gather quickly. The Adoption Backpack program is one way FIFK promotes stability and “normalcy” by meeting youth where they are and responding with care, dignity, and encouragement.

When submitting this form, you may be asked for basic information such as the youth’s first name or initials, age or general size needs, placement details (as appropriate), and delivery or pickup preferences. FIFK uses this information solely to prepare the backpack accurately and coordinate fulfillment. We encourage requesters to provide only what is necessary so we can complete the request while respecting privacy. If any details are unclear, our team may follow up to confirm the information needed to fulfill the request correctly and on time.

Reimbursement Form

The Reimbursement Form supports approved expenses tied to FIFK’s mission—helping ensure that youth and caregivers can access resources without the burden falling entirely on families, volunteers, or partner organizations. Reimbursements may cover eligible items or services connected to youth needs, normalcy experiences, or program support, depending on FIFK guidelines and available funding. This process helps keep our programs sustainable and transparent while still responding to real-world needs quickly.

To process reimbursements efficiently, the form typically requests essential details such as the requester’s name, contact information, reimbursement amount, a short description of the expense, and supporting documentation such as receipts. Clear documentation helps FIFK maintain strong stewardship of donor funds and ensures fairness and consistency across requests. If something is missing, we may reach out for clarification so we can finalize reimbursement without delays.
